Instructions for Independent Study Proposal Submission
If you have discussed the possibility of completing an independent study with an instructor, you must complete this form. Be sure to include the email address of the faculty member you intend to work with.
Once you attach your proposal and fully complete this form:

  • A copy will be sent directly to the faculty member for their review and approval.
  • After the instructor signs the form, it will be forwarded to the Associate Dean for final review and approval.
