Leave of Absence/Program Withdrawal Form

Academic Affairs:
It is your responsibility to confirm that all of your classes have been dropped. If you need assistance doing this, contact Academic Affairs. Your CUNYFirst account will be marked as “Deactivated/Discontinued” once your Leave of Absence is processed. It is advised that you update your personal contact information on your account before this occurs. When you do return, your account will be reactivated upon potential acceptance of your Readmission Application. If you have on-campus employment (work-study or college assistant), or are participating in or volunteering with a grant program, you are required to stop working immediately and to notify your on-campus employment supervisor or program director that you will no longer continue, as you are no longer a student.

Career Services:
All resources available through Career Services. This includes access to Handshake (jobs/internship postings, resources section), appointments with Career Services, and the ability to participate in fairs or other career programming.

Equipment Room:
The ability to reserve and check out equipment. If you have been issued equipment, you will be required to immediately return all issued equipment and pay any related equipment fines.. Please email the Equipment Room at o schedule a return time.

ID Card: The ID card will be deactivated along with the ability to occupy locker space.

IT: Applications including Adobe Creative Cloud, Google Drive, J-School email address, Microsoft Office, Pro Tools, Zoom.

Research Center: Access to the library database and resources shared via email.
