Insurance Requirements
An insurance plan specifically tailored to the program has been arranged with Haylor, Freyer & Coon Inc. (HFC). Policies have been purchased for each student by the Technology and Library Committee using student fee funding.
The standard Policy Period will encompass three semesters of the graduate program. Should the student enroll in additional coursework after this period, additional insurance must be purchased for continued equipment access.

First Semester
Students will receive Shure MV88+ Video Kits for use during their first semester. These kits will be used in conjunction with the students’ smartphones for required coursework.
In order to receive these kits, students will be required to:
– Schedule a Pickup in Google Calendar
– Pick up the Equipment Kit at the Equipment Window (Room 329)
Specific instructions and submission forms will be provided by Equipment Services via email
Second Semester
Students will be issued equipment kits determined by their coursework. These course-specific kits will be issued for the duration of the course and must be returned as directed at its conclusion.
Specific scheduling instructions will be provided by Equipment Services via email.
In the event that a student drops a production course after equipment has been issued, the student will be required to immediately return the equipment associated with that course.
Summer Break & Internships
Equipment may be checked out over the Summer Break for Capstone and Independent Study coursework, pending Advisor approval. Please contact Equipment Services for specific instructions.
Equipment may not be used to complete work assigned as part of an internship.
Third Semester
Students will be issued equipment kits determined by their coursework. These course-specific kits will be issued for the duration of the course and must be returned in full at its conclusion.
Specific scheduling instructions will be provided by Equipment Services via email.
In the event that a student drops a production course after equipment has been issued, the student will be required to immediately return the equipment associated with that course.
Additional Semesters
In the event a student enrolls in additional semesters, the student will be issued equipment kits determined by their coursework. These course-specific kits will be issued for the duration of the course and must be returned in full at its conclusion.
Specific instructions will be provided by Equipment Services via email.
Additional Equipment
A student will be considered to be in good standing for the duration of their coursework, pending submission of an active HFC Proof of Property Insurance Certificate and in the absence of any account holds.
A student will be placed on hold and restricted from equipment access in the event of non-returns, incomplete returns, non-submission of insurance claims, non-payment, non-communication, recurring damage to equipment or in the absence of an active HFC Proof of Property Insurance Certificate
Limits & Conditions
In the event of Equipment Loss, students will be responsible for the replacement cost of the missing items. Please note that the HFC Student Property Insurance does not provide coverage for loss.
In the event of Equipment Damage, students will be responsible for the replacement or repair cost of the damaged items. Please contact Equipment Services immediately to begin a repair and forguidance submitting an insurance claim to HFC.
In the event of Equipment Theft, students will be responsible for the replacement cost of the stolen items. It is critical that Police or CUNY Public Safety, depending on the location of the theft, be contacted immediately in such situations and that a report be filed. Please contact Equipment Services for guidance submitting an insurance claim to HFC.
Good Standing
A student will be considered to be in good standing for the duration of their coursework, pending submission of an active HFC Proof of Property Insurance Certificate and in the absence of any account holds.

Non-returns & Nonpayment
In the event of non-returned equipment or nonpayment of replacement or repair costs, the student’s equipment account will be placed on hold and after 30 calendar days the full replacement cost of such equipment will be charged to the student’s bursar account.
